Financial Policy

1) We Do Not Bill Insurance

We operate as a direct, self-pay practice. We do not submit claims to insurance, Medicare, or Medicaid. You may choose to self-submit; reimbursement (if any) is between you and your plan. We can cooperate with reasonable verification requests, but we are not a party to your insurance contract. To learn more about our insurance policy click here.

2) Good Faith Estimates (No Surprises Act)

You are entitled to a Good Faith Estimate (GFE) of expected charges upon request at any time. GFEs may include related items/services reasonably expected for your visit; actual charges can differ.

3) Prices, Estimates & What’s Not Included

Unless explicitly bundled in writing, fees exclude third-party costs (labs, specialty kits, imaging, outside providers), supplements, and devices. We will disclose pass-through items before charging. To learn more about our Financing options click here.

4) Payment, Card on File & Surcharges

Payment is due at time of service. We accept cash, credit/debit/HSA/FSA cards, and checks (checks accepted only with a valid card on file).

Card on File (required): To schedule/receive services, you authorize a tokenized credit/debit/HSA/FSA card to remain on file to settle:

  • Visit fees per your booking

  • Approved programs

  • Pass-throughs you authorize (e.g., labs/supplements)

  • Late/no-show fees (see Section 5)

 

Card data is stored by our PCI-compliant processor; staff cannot view full card numbers.

No surcharges / no “checkout fees.” We do not add any extra fee for paying by credit card, debit card, HSA/FSA, cash, or check. (Surcharging rules are complex and generally limited to credit cards; we simply don’t do it.)

Returned checks: $18 per returned check, plus bank charges.
Declines: You agree to maintain a valid payment method and resolve declines within 2 business days.

 

5) Cancellations & No-Shows

We reserve one-to-one time and often prep casework in advance.

  • 48+ hours’ notice: You may reschedule up to three times at no charge.

  • <48 hours: 25% of the scheduled fee.

  • No-show (including >30 minutes late without notice): 50% of the scheduled fee.

  • Repeat violations: We may require prepayment, limit scheduling, or discharge you from the practice.

 

6) Programs, Memberships & Retainers

Programs/memberships bill on the published schedule (e.g., monthly). You authorize recurring charges until the term ends or you cancel per program terms.

Case-work / report prep: When quoted and approved, retainer time is billable whether or not you attend a visit.

 

7) Refunds, Credits & Chargebacks

All sales are final once the cancellation window has passed. Billing errors (e.g., duplicate charges) are corrected promptly.

Chargebacks that conflict with signed policies, booking records, message logs, or chart/service notes will be contested.

 

8) Communication & Casework Boundaries

Portal messaging is for brief administrative needs. Clinical changes, new concerns, or substantive review require a visit or draw from any pre-approved case-work allotment.

 

9) Telehealth & Jurisdiction

Services originate in Ohio (even when you are remote). By engaging our services, you agree Ohio law governs agreements and disputes and consent to venue/jurisdiction in Geauga County, Ohio.
